My doctor back home owned other services. So he would send you for tests that possibly weren't necessary, but that he could bill the insurance company for. Tried a new doctor, he was worse. Face it, everyone has health issues at one time or another in their lives and the insurance companies dictate the doctor's practises and your care.
I just spent the past two years fighting an insurance company to GET me care for a long standing injury, and I can tell you; it has one of most frustrating and demoralizing periods of my life. The female lawyer and doctors representing that company are in bed together and the object is NOT to treat you unless you have a court order. Our system is entirely flawed, no matter what Tom says.
