I'll comment on one item: pay per service (6a) is a big part of the problem of health care costs now. 80% of health care costs are in treating just a few diseases. Instead of paying every time a patient walks in the door, averaging the costs of treating these diseases over a patient's lifetime (diabetes for example) and covering treatment based on that number would dramatically reduce administrative costs and save hundreds of billions over the long run.

Unfortunately this idea has gained little support from democrats pushing through health care reform.
