The overall requirement is a form of closure that helps friends and loved ones cope. Within that range are many options.

I don't know about Muslims. I suspect that over the centuries special and approved exceptions have evolved for the many possible conditions where a formal ground burial is not possible. For example, I know there are rules for burials at sea when a land burial is infeasible.
