It seems like it would work the same way as say, a truly free market in cod-fishing.
Keep on plugging away sucking the life out of your market (doctors or cod population) until either:
Your product is dead (Doctors move out of state, a la PA, or cod reduce to below a self-sustainable population)
or
The government steps in (and imposes tort reform*, e.g CA in past, PA soon, or fishing quotas**).
Or does this whole analogy smell, well, a bit, um, fishy?
