The issue is not centralization or decentralization. You are aware that insurance companies could exist quite nicely without government force? Maybe there's a language issue, but "collectivization" and "centralization" are not the same thing.
Do I get to choose to buy insurance or not? Do I get to select from several companies? Do I get to pick alternatives to standard insurance? Can I make my own choices without falling afoul of the law?
Or do I get fined and/or jailed if I don't buy insurance? Do I have to buy my earthquake insurance from a state-controlled monopoly? Both of these are true already for me.
Will I someday have any other choice for providing my own health care other than the Federal government of the United States?
The issue is not organization. The issue is the gun. I don't understand why this is so hard to get across.
