Without going into specific ideas, any reform needs to have, as a long-range goal, re-introducing market forces and competition into the system. Right now these forces are practically dead, both from massive government support of medical care and from insurance companies that cover almost everything. Americans are already trained to believe that if they have to write a check for medical care, no matter how small, they are being seriously abused.
We are not going to accomplish that in a year, or ten years. One of the problems with freedom vs. statism is that you can impose statism in a week. It takes many years to pry a major government program loose from the population.
Your choice number 2 is not unworkable, and it would be a lot cheaper than number 1. Programs to underwrite the "uninsurable" already exist in many states, and for the time I was buying my own insurance my wife was under such a "high risk" program in California. I do not have problems with government programs that assist those in real trouble, and California spent far less on that program than they would have insuring every last Californian.
