Where were the 1 megaton tests done on the surface in the US? Even in the Pacific, the tests caused fallout which poisoned and burned islanders and fishermen.
What you're missing or ignoring is the fact that the fallout would be blown over some of the most populated regions of the world. Regions where due to the modest living standards, people are in direct contact with the environment everyday-all-day. No hopping into the SUV and breezing off to the mountain home till it dissapates.
