A lot of things that aren't right still happen in America by Americans. We are a country with fallible people, diverse in nationality and nature.
"Isolated abuses" like 16,000+ murders a year is sure testament to that and even though we all know better there are a few who break the rules, just like the breaking the rules of torture.
And some, you would never expect to commit that horrendous crime.
The good news is, we care. Most of us anyway and that's how I look at the torture problem.
