I haven't done an exhaustive study of the current top 400, but here's an article on the list in 2007.
Nobody's been "squeezed" to make these people rich. They produced things people want to buy, they employ millions, and create the real wealth and products that make everyone prosperous. Microsoft alone has helped create something like 15 million jobs around the world in Microsoft-related industries. Half of them in the U.S.
Most of these people earned it. Hell, even Michael Moore earned his money in a free market.
Your example of the businessman who cut his own salary to keep his employees is not rare. I worked for one myself.
