If the guy was homeless and broke, I would take him to the hospital where my wife works. Like many hospitals, it is a non-profit organization but it has to take in more money than it loses to survive.
And he would get treated at the hospital regardless of whether or not he was covered by anything at all. More than half of all such care is paid for or written off by the hospitals themselves, not reimbursed by Medicaid or anything else.
"Government" is a broad term. My city has a government. My county has a government. My state has a government (it sucks, but it's there). There are many, many non-profit organizations and groups at all levels of society that are not government at all.
What sets off my "fascism" alarm is when people will accept no other solution except a monolithic one that is imposed on the entire nation by our overlords in Washington D.C. And Washington D.C. is out of money. It is easy to tell that this has not yet sunk in on a number of levels.
