Medicaid is a state-run program, not a Federally-run one, although over half the funding (it varies by state) comes from the Federal government. No doubt states would dearly love to see that go to 100 percent, since the Feds can print money and they can't. State participation was voluntary, and the last state, Arizona, came in about 1982.
