It is possible to explain a lot by simply realizing that some people lack initiative. It has nothing to do with race, intelligence, or anything else. I don't know what the root causes are. But some people seem to lack a belief that one can change one's circumstances in life if you simply decide to do something about it.
It rubs some people the wrong way to imply that any problems at all in this world just might be a person's own fault, but that doesn't mean it can't be true.
The fact that people without initiative can also be counted among the very poorest would not surprise me, although I understand this rubs people the wrong way too.
Obviously, since most of the poor seem to have no trouble getting medical services (according to the paper), lack of initiative cannot be the only cause of poverty, and I'm not saying it is.
