The difference is a private company has to earn more money than it spends. The government can just keep borrowing it, or raise taxes. There is little incentive to control costs.
The only way medical costs will come under control is if at least some of the costs are borne by the users. Supply and demand. Without market controls, rationing will be the only other recourse, and when rationing is done by the State, it is controlled by those with political pull.
There is already evidence that increasing the amount of costs that the end user is responsible for has already slowed the increase of medical spending in areas where this has been done. It remains to be seen how much of this is people not doing unnecessary things, or people not doing things they really need. This needs to be studied.
