Those are over and above what the insurance would call "usual and reasonable" charges and therefore will not pay and you can not be billed for them.

I have a friend who had brain cancer and had surgeries and multiple hospitalizations. She had no insurance and her bills were astronomical. They should not be able to bill the uninsured $1 more than what insurance would pay.
