The doctors will have to justify their actions to the people who cough up the money, just like they do now. That it's a government bureaucracy instead of an insurance company won't make any difference. The paperwork for Medicare/Medicaid right now is at least as bad as the paperwork for private insurance companies, and when there's only one legal provider, guess who gets to call ALL the shots.
